Integrated Care for Kids (InCK)

The Integrated Care for Kids (InCK) project is a child-centered service delivery and payment innovation model that aims to improve the quality of care and reduce expenditures with a focus on children under 21 years of age covered by Medicaid through prevention, early intervention, and treatment of behavioral and physical health needs. The model is funded by the Centers for Medicare and Medicaid Services and is being implemented in seven different regions across the country.

The Ohio InCK project is led by Nationwide Children's Hospital and the Center for Child Health Equity and Outcomes Research. In partnership with local providers in Licking and Muskingum County, the project involves:

  1. Needs assessment and risk stratification – a population-level approach to assess for risks spanning physical health, behavioral health, and health-related social determinant needs
  2. Early identification and prevention/intervention services – introduce, sustain, or expand core child services that address needs within the population
  3. Integrated care coordination – care management and care coordination services for children with moderate and high-risk levels
  4. Value-based payment – aligning service delivery with financial incentives linked to the project's desired health outcomes

David Ciccone is project manager in the Center for Child Health Equity and Outcomes Research with responsibility for operations of the Integrated Care for Kids (InCK) project. Dave has been involved in community and health improvement work for most of his career. Prior his work at Nationwide Children's, he’s held leadership roles in nonprofit management and philanthropy at several central Ohio organizations. Dave graduated from The College of Wooster with a bachelor's degree in psychology and minor in philosophy.

Miriam Ali is a public health professional, who received a masters from Kent State University and is currently pursuing a doctoral degree from Franklin University. Miriam's passion lies in making positive changes in the well being of communities particularly by addressing maternal and child health disparities.

Jacqueline is a case management extender for the Integrated Care for Kids model in the Chisolm Lab. She has experience in direct patient care in an inpatient behavioral health setting, and case management for behavioral health and substance use recovery populations. Her current role consists of implementing outreach to patients and families to coordinate eligible referrals to the Integrated Care for Kids care coordination program and assisting families with navigating and accessing community resources.

Tina Brown is a service integration coordinator for the Integrated Care for Kids (InCK) project. Tina graduated from Capital University with bachelor of social work. Prior to coming to Nationwide Children's Tina worked as a children services caseworker for 17 years. She also was a case manager at other agencies in Ohio, prior to working as a children service worker.

Rachel Bruck

Rachel is a service integration coordinator for the InCK team. Rachel graduated from Capital University with a bachelor’s degree in nursing in 1994 and Western Governor’s University with a master’s degree in nursing education. She has been a registered nurse at Nationwide Children’s Hospital for 29 years working in the PICU, In-House Agency Float Pool, leadership as a clinical leader, and a nurse educator for the Behavioral Health Pavilion. Rachel has a passion for behavioral health and loves engaging families while connecting them with care coordination.

Erin is the clinical systems coordinator for the Integrated Care for Kids (InCK) model, situated within the Center for Child Health Equity and Outcomes Research. In this role, she leads a team of nurse and social worker service integration coordinators and helps develop and advance the care coordination aspect of the project. Erin has spent the entirety of her nursing career at Nationwide Children's Hospital, where she has held previous roles in unit-based pediatric behavioral health and nursing leadership. She completed her master’s of public health in 2022.

Amy is a registered nurse with a passion for maternal child health and had the privilege to work in direct patient care for the majority of her career. She received her bachelors of science in nursing from Mount Carmel College of Nursing in 1994. As a member of the InCK team, Amy works on a multi-disciplinary team that is responsible for assisting in the development and implementation of the Integrated Care Management Model and Care Planning Team Model as part of the Integrated Care for Kids grant award.

Stacey is a registered nurse by trade as a second career in her life, her master’s degree is in business and then she went back for a nursing degree after her children were born. Stacey has since combined nursing with business and has been in leadership roles in the hospital before joining the InCK team more than two years ago. In this role as a service integration coordinator, Stacey assisted in developing the model during the planning phase and has since been outreaching community families to provide necessary care coordination during the implementation phase. Stacey is also a member of the LEAD Professionals group at Nationwide Children’s Hospital and is a member of Children’s and Family First Leadership program.

Kaitlyn is a service integration coordinator for Integrated Care for Kids (InCK). She graduated from The Ohio State University where she earned her bachelors of science in nursing. She started her career working as a nurse on inpatient units at The Ohio State University Wexner Medical Center, but most of her career has been spent working in community and public health nursing organizations. She is a Southeastern Ohio native, so she is especially passionate about coordinating care and services for children and families living in Muskingum and Licking counties.

Christina is a service integration coordinator (SIC) for the Chisolm Lab InCK program. She graduated from Ohio University in 2023 with her master of science in nursing. Christina has experience working as both a bedside nurse and care coordinator for women's health, NICU, and behavioral health patients. Christina's current role provides the opportunity to assist families in utilizing the behavioral health and community resources that are available to them. The role of a SIC is to support families and assist in getting them involved with care coordination. Care coordination is essential in helping families address and prevent gaps in care. The purpose of this work is to link families with the providers and services they require in effort to decrease out of home placements for the children she serves.
