Integrated Primary Care Clinic

A core component of the IPC internship is learning how to integrate into a medical team. During the course of the year, the intern will provide consultation to patients and families presenting for primary care appointments to provide same day services for a wide variety of presenting concerns ranging from procedural distress, ADHD, sleep concerns, feeding concerns, family conflict, depression, anxiety, developmental concerns, and weight management. The primary care team at Nationwide Children's Hospital serves racially and ethnically diverse populations and provides care for individuals aged 0-21 years.

Outpatient Therapy

The IPC intern will take a caseload of patients referred for co-located primary care psychology services. The intern will provide targeted behavioral health care for children and adolescents with a wide variety of presenting concerns including disruptive behavior, depression, anxiety, adjustment, elimination disorders. Treatment experiences include brief individual, child & parent, and family consultation and intervention, brief outpatient intervention and group facilitation, and community support programs unique to our primary care network. Additionally, the intern will have opportunities to engage in outpatient therapy through secondary rotations, such as through the outpatient feeding or adolescent medicine program.

Psychological Assessment

The intern will complete psychoeducational evaluations (goal of 3 within a 6-month rotation) throughout the course of their year-long internship for children and youth ages 6-18 years referred for a psychoeducational evaluation in order to provide diagnostic clarification and/or rule out intellectual disabilities, learning disabilities, ADHD, and other psychiatric diagnoses that may be impacting the patient’s functioning. Interns will administer, score, and interpret cognitive assessments, achievement assessments, broad-based rating scales, and symptoms-specific rating scales. The intern will write a report summarizing results, diagnoses, and recommendations. The intern will also provide feedback to the patient and caregivers regarding diagnoses and recommendations.

The intern will also complete targeted ADHD evaluations (goal of 3 within a 6-month rotation) throughout the course of their year-long internship for children and youth ages 4-20 years referred by their primary care provider due to concerns for complex ADHD. The intern will administer, score, and interpret broad-based and symptom-specific rating scales to provide diagnostic clarification. The intern will write a brief report summarizing results, diagnosis, and recommendations. The intern will also provide feedback to the patient and family and collaborate with the primary care team and family to determine treatment recommendations.

Supervision

The intern will receive both individual and group supervision from psychology faculty members specific to their activities. Supervision is a source of pride for our hospital and an area of continued dedication to education and growth for our clinical faculty. The IPC intern receives supervision from three core faculty, allowing for exposure to different styles, approaches, and specialization of content. Supervision is provided from a developmental perspective and is focused on building confidence, self-assessment and reflection, individual competencies, professional identity, independence, and career shaping. Group supervision experiences include monthly case conference meetings with the primary care psychology team. Individual supervision for integrated consult services is both structured/scheduled and on-the-fly.

Research

Due to the clinical focus of the internship, research experiences are not a core part of the training experience. However, for interns who have defended their dissertation, research opportunities are available. Additionally, there are several Quality Improvement / Process Improvements projects ongoing. These projects are aimed at increasing equitable access to behavioral health care and contribute to the broader scholarship in integrated primary care. Interns are also expected to consult the relevant literature in their clinical work and share research knowledge with colleagues.

Didactics

Didactic experiences in the IPC Psychology track are both formal and informal. The IPC intern will attend approximately 2 hours of didactic training per week, many of which will be with their fellow interns in pediatric psychology. Didactic experiences are outlined below.

  • Nationwide Children's Hospital Internship Seminar with all Nationwide Children's Predoctoral Psychology interns – 90 minutes/week
  • Research Seminar with pediatric psychology interns – 1 hour/month
  • Clinical Service Hour with pediatric psychology interns – 1 hour/week
  • Pediatric Specialty Seminar with pediatric psychology interns – 1 hour/week
  • Diversity, Equity, and Inclusion Seminar with pediatric psychology interns – 2 hours/month
  • Journal Club with integrated primary care team – 1 hour/month

Many additional educational opportunities exist within our hospital setting including Medical Grand Rounds, Tumor Board, Clinical Research Seminars, Schwartz Center rounds, and the Multicultural Brown Bag Lunch Series.

Hours

Integrated primary care interns work five days per week and are expected to see clients two evenings during the week. There is no “on call” coverage and interns typically work 40-50 hours per week.
