Celebrating Home Care, Hospice and Palliative Care Month

Homecare and Hospice is celebrating growth and best outcomes during the month of November. The Fall Quality Improvement (QI) Report highlights the home medical equipment (HME) QI study and enhanced performance delivery. Additionally, Homecare’s clinical staff worked with a multidisciplinary team to implement QI initiatives aimed at reducing incidence of tracheitis, beginning in 2016. This resulted in a 70% improvement in tracheitis rates, which has decreased and remained stable through the second quarter of 2024.

Homecare’s services continue to serve more patients in more counties throughout Ohio. HME services are available in all 88 counties, except for phototherapy. Territory expansion in 2024 and 2025 includes:

Skilled Therapies

  • 2024: Fairfield County
  • 2025: Licking County

Intermittent Skilled Nursing

  • 2024: Knox, Muskingum
  • 2025: Perry, Hocking

Skilled therapies and intermittent skilled nursing are also now available in the Toledo area (Wood, Lucas and Hancock counties).
