Homecare Recognitions and Reminders

In March, Homecare celebrated three critical disciplines: social workers, child life specialists and musical therapists. Hospice and Palliative Care patients, as well as other Homecare complex patients, rely on these services daily. The crucial assessments and interventions provided by these disciplines support our staff, patients and families daily, resulting in best outcomes. Social workers, child life specialists and musical therapists enable the patients they care for to live their best lives in the comfort of their homes.

The warmer days of spring bring increased pollen, often triggering asthma flare-ups. Homecare’s Asthma Express continues to provide care and instruction in the homes of patients who are newly diagnosed or experiencing new severe flare-ups that have led to emergency department visits or inpatient stays. Home visits by Asthma Express nurses help patients manage their asthma, keeping them in school, at home and out of the hospital. For more information, click here.

Finally, for providers submitting referrals for incontinence supplies for patients, please note that all incontinence Certificate of Medical Necessity (CMN) require two diagnosis codes. See the incontinence supply CMN insert for details.
