School Health Services at Groveport Madison Schools
Does your child need a yearly checkup? Are they feeling sick? Do they seem anxious or depressed?
Groveport Madison Schools students and all community members, ages 0+, can get health care at the Nationwide Children’s Hospital School-Based Health Center, located at the Groveport Madison High School.
Available services include primary care, dental care, sickle cell support, diabetes services and asthma therapy. If you need help getting to and from the school-based health center, Nationwide Children's also offers transportation services.

How to Schedule an Appointment
Primary Health Care Services
There are four ways to schedule primary health care services:
- Visit the center between classes or before and after school, if you have a center in your school.
- Call to schedule: (614) 355-2590, option 3.
- Ask your school nurse to help you schedule an appointment.
- Complete the consent form and someone from our team will call to schedule your appointment.
Location and Hours
The school-based health center is open during the school day. Hours may vary.
- Primary Care - Open 7:30 a.m. to 4 p.m.
- Dental care is also available on select days.
Groveport Madison High School’s Campus
To access the school-based health center, start at the main entrance of the high school parking lot (4475 S. Hamilton Road) and follow the directional signage until you reach the Nationwide Children’s parking lot.
Services Available to Groveport Madison Schools Students and Community Members
- Primary Care: Nationwide Children’s provides access to health care for children, ages 0 to 21, in schools or on a mobile care center. Services include checkups, sick visits, vaccines, COVID-19 testing, medication management, blood work and more. For more information, contact SchoolHealth@NationwideChildrens.org.
- Dental Care: Nationwide Children’s staff offers comprehensive dental services to prevent and repair tooth decay, pain and infections. Dental care includes cleanings, fluoride treatments, oral exams, x-rays, sealants, and more.
- Diabetes and School Health Program: The Diabetes and School Health (DASH) Program provides direct oversight, education and support for students with Type 1 diabetes in school. For more information, email DASH@NationwideChildrens.org.
- Asthma Therapy: The School-Based Asthma Therapy (SBAT) Program is a partnership between the school nurse and medical provider to ensure students with high-risk asthma receive their medication every day by taking it at school. Contact SBAT@NationwideChildrens.org for more information.
- Sickle Cell Support: The Sickle Cell OUTreach in Schools (SCOUTS) Program supports students with sickle cell disease by improving medication adherence, bridging gaps in care and providing education to schools and families.
- Transportation: Groveport Madison will provide transportation to and from the school-based health center for appointments. Staff will work with you to coordinate the necessary transportation services for your student based on their appointment time.
The services provided are not meant to replace your pediatrician or family doctor. When you have any medical need, your first call should always be to your pediatrician or family doctor who knows you best. No child will be denied care due to inability to pay for services. We accept most insurance plans, including Medicaid.
For Parents
Parents and guardians are welcome to come to their child’s appointment, but it is not required. Our providers can see your child as long as you have provided consent.
When your child sees the nurse practitioner, staff will attempt to reach you to provide an update by calling the number on the consent form. Additionally, a document called an After Visit Summary (AVS) will be sent home with your child. The AVS will describe the services provided and any recommended follow-up, if needed.

One of the most common childhood diseases is tooth decay, which can result in pain and infection. Our school-based dental services help make sure students' teeth stay healthy and strong.
