Nationwide Children's Hospital accepts patient referrals and appointment requests through several options:
- Complete our online Request An Appointment form
- Call Central Scheduling at (614) 722-6200 or 1 (877) 722-6220
- Fax a referral request to (614) 722-4000
When referring a patient to Nationwide Children’s, please include the following with the referral information:
- Pertinent question.
- Patient history/PE with most recent/pertinent office notes.
- Pertinent labs/radiology studies. If not done at Nationwide Children’s, please send the radiology images and lab results as an attachment.
- Most current growth curve.

Can my patient be seen sooner?
For referring providers who already have an appointment for a patient, who needs to be seen sooner and it's not an urgent case, we recommend you call Central Scheduling to inquire about rescheduling.
If needed, you may call our clinics directly and schedule accordingly.
