The services provided are not meant to replace your pediatrician or family doctor. We accept most insurance plans, including Medicaid.
What to Expect at Your First Appointment
Whether your appointment is at school or a virtual appointment at home, we want to make sure your appointment runs smoothly!
Here are a few things you should know.
Consent
- We’ll need a parent or guardian to provide consent for your first appointment. Copies of the consent forms are available in the “For Parents” tab on this page.
- If you are requesting behavioral and mental health services, we’ll need a parent or guardian to meet in-person to complete consent and intake paperwork, diagnostic assessment, and treatment plan to start services.
Parent/Guardian Involvement
- Parents/guardians are welcome to come to an appointment, but it is not required as long as they provide consent.
- When you see a nurse practitioner, staff will attempt to reach your parent/guardian using the number provided on the consent form.
- We’ll also give you a document, called an After Visit Summary (AVS) for you to share with your parent or guardian. The AVS will describe the services provided and any recommended follow up, if needed.
- If your visit is with a behavioral and mental health clinician, the clinician will schedule family sessions and/or phone calls with your parents/guardians to provide updates on progress and to seek input on treatment decisions.
