Partners For Kids’ Asthma Toolkit

Partners For Kids has designed a toolkit to help health care professionals address the needs of their patients with asthma. The toolkit includes a list of resources for providers and patients, including prescribing guidelines, educational webinars, action plans, tips for inhaler usage and more – all in one place. The information is updated regularly based on current clinical guidelines, recommendations and data.

Food Resources for Families

Help families find free resources, including places to get meals, walk-in food pantries, food stamp (EBT) assistance and more through CAP4Kids. CAP4Kids is a website that provides many activities and resources to help families in the central Ohio area. Check out their food resource insert card to share with patient families.

Project ECHO Educational Programming

Project ECHO (Extension for Community Healthcare Outcomes) is a collaborative and innovative tele-mentoring program designed to create virtual communities of practice by bringing together subject matter experts and community providers.

Partners For Kids’ Care Coordination Awarded NCQA Accreditation

Partners For Kids’ Care Coordination was awarded a three-year accreditation status upon completion of the NCQA (National Committee for Quality Assurance) Complex Case Management accreditation survey. This is the highest category awarded by NCQA for service and quality, which demonstrates strong performance of rigorous requirements for case management.

The NCQA is a central figure in driving improvement throughout the United States health care system and is recognized as the highest standard of accreditation and validation of health care programs.
