Qualifications for Appointment and/or Clinical Privileges

Every applicant who applies for Medical Staff Appointment and/or Privileges, or for Privileges (such as temporary, locum tenens, disaster, TeleHealth/TeleMedicine), must at the time of application and Initial Appointment/Privileging and continuously thereafter, demonstrate to the satisfaction of the Medical Staff and the Board that he/she meets all of the following qualifications and any other qualifications and requirements as set forth in the Nationwide Children’s Hospital Medical Staff Bylaws and Policies, the Hospital’s Rules and Regulations and Patient Care Policies, or as otherwise hereinafter established by the Board.

  1. Have and maintain a current, valid license/certificate to practice in the State of Ohio.
  2. Have and maintain a current, valid Drug Enforcement Administration (DEA) registration number.
  3. Provide documentation of completion of professional education as required by the applicable State licensing entity and such additional education as may be set forth in the applicable Delineation of Clinical Privileges.
  4. Provide, if applicable, documentation of successful completion of an ACGME and/or AOA approved residency in the specialty/specialties in which the applicant seeks Privileges. Applicants shall also provide documentation of successful completion of other postgraduate training programs, internships, and/or fellowships, as applicable, with specification as to pediatric training.
  5. Provide documentation of board certification and maintenance of certification in his/her area(s) of practice at the Hospital by the appropriate specialty/subspecialty board(s) in accordance with the requirements set forth in the Credentials Policy.
  6. Have and maintain current, valid Professional Liability Insurance in an amount no less than one million dollars ($1,000,000) per incident and three million dollars ($3,000,000) annual aggregate.
  7. Be able to participate in Federal Healthcare Programs.
  8. Have not been convicted of or pled guilty to any of the violations of Medicare, Medicaid, or other federal or state governmental or private third-party payer fraud or program abuse, nor have been required to pay civil penalties for the same.In the event an applicant seeks to request a waiver of this qualification on the grounds that the applicant meets the rehabilitation standards as provided for in R.C. 109.572(A)(4), the applicant shall follow the waiver procedure set forth below.
  9. Criminal Background Check: Complete such documentation as is necessary in order that the Hospital can perform a criminal background check.

Waiver

If you do not meet one or more of the criteria, you may request that it be waived.  If requesting the waiver, you would bear the burden of demonstrating that your qualifications are equivalent to, or exceed, the criterion in question.  A written request for a waiver for a qualification for Medical Staff Appointment and/or Privileges may be submitted by the Practitioner for consideration by the Medical Executive Committee (MEC) and Board.

Credentialing/Initial Appointment/Reappointment/Privileging

The process for Credentialing, Initial Appointment, Reappointment and Privileging shall be as follows:

  1. In order to become a member of our Medical Staff, you must submit a completed application.Your completed application shall be submitted to the Medical Staff Services Office who shall review each application for completeness and perform primary source verification.
  2. We utilize the Council for Affordable Quality Healthcare (CAQH) application as part of the credentialing process, which according to H.B. 125 of The Healthcare Simplification Act, is the only acceptable application for physician credentialing and reappointment in the State of Ohio.You can find information on the CAQH application process at www.caqh.org.
  3. There is a $200 application fee.We must receive your application fee along with your Medical Staff Application before it can be processed.Please note that the application fee is a one-time fee and is non-refundable once the process has started.After Initial Appointment, there is an Annual Medical Staff Dues fee of $200.

  4. The detailed procedures, with respect to APPs, for credentialing and for the delineation, granting, and regranting of Privileges are outlined in the APP Policy.

Frequently Asked Questions

How long is the application process?

The application process takes 60-90 days.

The verification process takes approximately 45 days once the Medical Staff Services Office has received your 100% COMPLETED application.

How much is the credentialing application fee?

$200 for medical staff

If I decide I no longer wish to apply, is my fee refundable?

No. Once the verification process begins on your application, the fee cannot be refunded.

What is the CAQH and how do I complete it?

The CAQH is the “Council for Affordable Health Care” application. We utilize the CAQH application as part of the credentialing process, which, according to H.B. 125, The Healthcare Simplification Act, is the only acceptable application for physician credentialing and reappointment in Ohio. How often does the Credentials Committee meet?

Monthly – the third Thursday of every month.

Does the Medical Staff Office expedite applications?

No. It is not the policy of the Medical Staff Services office to expedite one application over another. Applications are processed in the order they’re received.

Does Nationwide Children's offer temporary privileges?

Yes. Providing you have interviewed with your Department Chair, your application is approved by the Credentials Committee and there are no concerns with your application, you will be sent a letter confirming you have been granted “temporary” privileges immediately following the Credentials Committee meeting. Permanent privileges are granted once your application has been presented to the quarterly Credentialing Board of Directors for approval, if applicable.

When can I start seeing patients?

Once you receive your “temporary” privileges letter OR once you receive your final approval letter from the Medical Staff Services office.
