Transition Program

Meet Michaela. A patient since seven years-old, Michaela discusses making the transition to the CF adult program now that she is 19.

The Cystic Fibrosis Transition Program is designed to help children and adolescents with cystic fibrosis develop the skills needed to successfully transition into being healthy, independent adults.

Goals of the Cystic Fibrosis Transition Program

  • To promote self-management
  • To promote self-motivation in adolescents
  • To teach adolescents adult life skills
  • To teach parents of children with cystic fibrosis techniques in promoting self-management of care
  • To promote ongoing education of health care providers regarding transition of pediatric patients to adulthood
  • To help parents/caregivers transition into a new supporting role
