Homecare Updates for Providers

RSV/Synagis Season: Providers, please visit our Synagis program page for 2022-2023 updated materials for you and your patients. Providers using Epic, search Homecare and open the “What’s New” tab for information on sending your referral to Homecare via Epic. Synagis Referral Checklist sheet for Epic and non-Epic users is available for providers on our website.

Signed Orders Request: The timeliness of provider signatures on Plans of Care (POC) continues to be a hurdle for successfully completing patient referrals. Whether Homecare receives a referral via phone, fax or Epic, a POC and order with the provider’s signature must also be sent to complete the referral. For providers not on Epic, fax signed order to (614) 355-1182. If a POC is not included at the time of referral, the individual order will be sent to the provider’s ‘In Basket’ for review and signature. Signed orders/POCs are always needed to schedule and provide ongoing patient care, accurately bill insurance and remain Medicaid compliant. We appreciate referring providers working with us as One Team to make this happen.

Asthma Express: Fall can mean increased allergens in the air and involvement in sporting events with back-to-school activities. Both can be triggers for more frequent asthma episodes in children. Homecare’s in-home asthma management program provides education and support to patients and their families. The Asthma Express Program helps children manage their asthma and keep episodes from becoming acute. Fact sheets for providers and families are available on our website.

Ohio’s ONLY Pediatric Home Care Conference on October 14: This conference provides value-added content for clinicians and administrators who work with pediatric patients with acute or chronic health care needs that require additional supportive care at home.
