Assessment Clinic
The Assessment Clinic at the Center for Healthy Weight and Nutrition is the first step for children and teens who struggle with being overweight. All patients must go through the Assessment Clinic. This helps to determine their health status and learn the right path to help them reach a healthy lifestyle as a way to improve their physical and emotional health.
The criteria to enter the Assessment Clinic is:
- BMI of 95% or higher
- Need to calculate a child's BMI? Use this BMI Percentile Calculator for Child and Teen provided by the CDC. If a child is more than 300 lbs. or if you receive a message saying "Please check the accuracy of the information you entered.", then please use this Adult BMI Calculator.
- Patient must have an established Primary Care Provider
Once a referral is received, the Center for Healthy Weight and Nutrition will call you to schedule an assessment visit. If the Center does not get a reply from you after one attempt then a letter will be sent to your home. You will then be asked to call our scheduling team at your convenience to schedule an appointment. These appointments are for new patients as well as follow-up appointments for existing patients.
New Patient Visit
The patient and their family will meet with a Doctor or Nurse Practitioner for a complete medical exam. Patients will be asked to not eat or drink anything in the morning so that they can visit the lab and have blood drawn. This appointment will take 1 hour and can take place at Livingston Ambulatory Center, Hilliard Close To Home Center, or our Westerville Close To Home Center. As you leave the appointment you will be able to schedule Assessment 2.
Follow-Up Visit
The patient and their family will meet individually with a Doctor/Nurse Practitioner, Registered Dietitian and a Physical Therapist. Patients will be asked to discuss their eating and physical habits. This appointment will be held at Livingston Ambulatory Center, Hilliard Close To Home Center, or our Westerville Close To Home Center and will take 90 minutes. The patient will be asked about their commitment and goals.
By the time the patient has completed the two assessment clinic visits, they will have decided on the best option for weight management in the Center - whether it is follow-up visits as needed, participation in one of our New U Programs or in our bariatric surgery program.
