Main Campus patients: Due to construction please allow extra time for travel.

Main campus patients: To limit exposure please call the office at 614-722-6717 when you arrive in the parking garage. 

Call the clinic today if:

  • The patient has symptoms of COVID-19 such as fever or cough
  • The person accompanying the patient or anyone in the patients household tested positive for COVID-19 in the past 30 days
  • The patient is currently under quarantine by the health department (includes notices by school/daycare, patients workplace) for COVID-19 exposure
  • The patient tested positive for COVID-19 in the past 10 days

Parent or legal guardian must accompany the patient.

Please bring any medications, vitamins or herbal supplements the patient is currently taking with you to the appointment.

Please arrive 15 min early for registration, if you are more than 15 minutes late, we may ask you to reschedule. 

If you cannot keep your appointment please call 722-6200 to reschedule.  This allows us to see another patient at this time.  If you come 30 or more minutes early, we will see the patients scheduled before you first.  We cannot promise you will be seen any earlier than your scheduled time.

Your Insurance benefits may include you are responsible for an  Office Visit co-pay.  Co-pays can be paid at the time of the office visit, we accept credit cards, debit cards and checks.
