At Nationwide Children’s Hospital, the health and safety of our patients, families and staff has always been our top priority and it continues during this COVID pandemic.

- Any visitor accompanying the patient to an appointment should be feeling well, should not have a recent exposure to COVID-19, and should not be under isolation for a positive COVID-19 test
- Complete the eCheck-in (Registration process) in MyChart on your phone or device.

Call the clinic today if:
- In the past 10 days, the patient tested positive for COVID-19
- The patient is under quarantine due to close contact with someone who has COVID-19? (Includes notices by the health department or exposures occurring in the household, school/daycare, workplace, or other community setting.)
- In the past 24 hours, the patient has had symptoms of COVID such as fever or cough
Please save this message until after your appointment because it includes instructions for the day of your appointment.
- All patients must be accompanied by a parent or guardian for the initial evaluation. Guardians who are not biological parents need to present documentation or proof of guardianship.
- What to Bring:
- Insurance card
- Medical Records from another facility if applicable
- Completed forms enclosed with packet
- List of medications
- IF YOUR CHILD RECEIVES SPEECH-LANGUAGE THERAPY
- Please bring copies of any recent speech therapy notes, evaluation reports, and your child's IEP (Individualized Educational Plan) to the appointment if possible.
- What to Expect
- Please plan to arrive 15 minutes before your scheduled appointment time for parking and registration.
- Your child may see multiple specialists during this visit which could require you to go to other departments located in the Ambulatory Building. We will direct you where to go when you arrive.
- Based on the number of providers your child needs to see, your visit may take 2-4 hours. Please be prepared with snacks or other necessities you may need.
