A parent or legal guardian must accompany the patient, or bring proof of legal guardianship.
Please bring proof of insurance. Also check with your insurance provider to see if prior authorization is needed. If you are responsible for an Office Visit co-pay, these can be paid at the time of the visit. We accept checks, credit cards and debit cards.
Please bring the following items with you to the appointment:
Medications, vitamins or herbal supplements. Bring a list of refrigerated medications (with dosage).
Please bring a photograph (head/shoulder) of your child with their name and date of birth on the back of the picture. This will be placed in your child's chart.
The patient should wear shorts underneath their clothing (for examination purposes).
Please arrive 15-20 minutes prior to your scheduled appointment. If you are more than 20 minutes late you may be asked to reschedule.
This appointment includes a visit with our pediatric psychologist, Dr. Alan Leever, which could lengthen your overall visit.
Because we are a teaching institution, you may be seen by a medical trainee in addition to your practitioner, which could lengthen your overall visit.
