MAIN CAMPUS PATIENTS:
If you are experiencing any fever, cough, nausea, vomiting, or diarrhea we would ask that you reschedule this appointment.
OFFSITE PATIENTS:
If you are experiencing any fever, cough, nausea, vomiting, or diarrhea we would ask that you reschedule this appointment.
Call the clinic today if:
- the patient has symptoms of COVID-19 such as fever or cough.
- the patient has been exposed to anyone with COVID-19 in the past 10 days.
- the patient tested positive for COVID-19 in the past 10 days.
Please save this message until after your appointment because it includes instructions for the day of your appointment.
- Please bring any pertinent information (x-rays, labs, immunization records).
- Bring copies of insurance card.
- Bring immunization record.
- Please bring any medications, vitamins or herbal supplements the patient is currently taking with you to the appointment.
- Parent or legal guardian must accompany the patient and must be present for entire visit. Guardians who are not biological parents need to present documentation of proof of guardianship, or child will not be seen that day.
- Foster parents must also bring a copy of the Medical Permit form provided by FCCS (Franklin county children services).
- Please arrive 15 minutes prior to scheduled appointment time.
- Your Insurance benefits may include you are responsible for an Office Visit co-pay. Co-pays can be paid at the time of the office visit, we accept credit cards, debit cards and checks.
