Once completed, for Main Campus appointments only please call registration at 614-722-5517 and let them know you are here.

OFFSITE PATIENTS:


Only one adult may be present for the appointment with child. No additional siblings or family members.

Call the clinic today if:

  • the patient has symptoms of COVID-19 such as fever or cough.
  • the patient has been exposed to anyone with COVID-19 in the past 10 days.
  • the patient tested positive for COVID-19 in the past 10 days.

Please save this message until after your appointment because it includes instructions for the day of your appointment.


  • Please arrive 15 minutes early for parking and registration.
  • A parent or legal guardian must accompany the child to their appointment.
  • Phototherapy appointments are quick treatments. If you arrive more than 10 minutes late for your scheduled appointment, you may be asked to reschedule.
  • Please bring the following
  • Insurance card (if you have one). Please call your insurance company prior to your visit to check if your visit is covered.
  • Photo ID.
  • All current medication(s) including prescription and over the counter medications, creams and lotions.
  • Form of payment for your co-pay. Co-pays are expected at the time of service. We accept personal checks, Visa, Mastercard, Discovery, American Express, and Debit Cards as a means of payment.
