Welcome Home

The Welcome Home project is a specialized program for teens transitioning from inpatient behavioral health care to outpatient care. The project provides teens with evidence-informed programming provided electronically through a combination of online tools and care coordination support.

Resources For Participating Teens

Now that you are coming home from the hospital, we want to help you keep up the progress that started while you were there. The Welcome Home Project gives you a chance to use two computer programs that have helped other teens cope with stress. These programs are “Recovery 4 Teens” and “COPE” (Creating Opportunities for Personal Empowerment). Welcome Home also includes other resources for dealing with stress, worries, and sad feelings.

COPE

COPE stands for Creating Opportunities for Personal Empowerment. It is a program that will help you learn to think differently and in ways that help you manage worries and sad feelings.

American Academy of Child and Adolescent Psychiatry Teen Resources: You can use this website to learn more about teen mental health. You will find videos of teens just like you talking about ways to cope, youth organizations, and other helpful information about teen mental health.

KidsHealth: For Teens: You can use this website to learn more about exercise and nutrition. You will find a lot of information about staying healthy, eating right, exercise tips and other helpful information about health and well being.
