During business hours, all frozen section notifications and communications will be made directly with the On-Call Pathologist. Use the On-Call Schedule ( http://intelliweb/ ) to page the On-Call Pathologist through the "Pathology/Laboratory" department, or call the NCH operator and ask to page the On-Call Pathologist.

All specimens will be hand-delivered to the frozen section room (J2342) within the OR complex. After hours, the procedure is to notify the pathologist on call (see above) prior to the procedure.

The specimen MUST be accompanied with a tissue requisition which includes all patient information (patient name, date of birth, medical record number, pertinent history, specimen source, and tests required). A Provisional Diagnosis MUST be provided as the indication for frozen section examination. Indication of culture necessity, if required, MUST be included.
