If specimen is for testing other than surgical pathology, please leave as fresh, do not place in formalin, and defer to the applicable laboratory guide to services entry for further instruction.

If formalin is unavailable, please contact Pathology (614-722-5450) during hours of operation (Monday-Friday 8:30am-5:00pm). During off hours, formalin and specimen containers are available in the OR Frozen Section room, J2342 (OR Procedures), or contact CPA Lab via Vocera "CPA Lab" or 614-722-5599 for assistance.

Main OR and GI personnel will deliver formalin specimens to the applicable formalin specimen drop-off locations, log in all specimens at the drop off site. Specimens will be delivered via CPA to Anatomic Pathology.

For specimens from other locations within main campus, call "CPA Lab" via Vocera or 614-722-5599, for a pick up.

All formalin specimen MUST be accompanied with Pathology Order via OpTime paperwork or surgical pathology requistion which includes all patient information (patient name, date of birth, medical record number, specimen source, pertinent history, and tests required).
