How to Request Medical Records

You can conveniently access the majority of your child's medical record without making a formal written request. Simply log in to your child's MyChart account today.
Nationwide Children's Hospital (Columbus) Medical Records Requests
Copies of medical records can be requested in one of five ways:
By MyChart:
Simply log in to your child’s Mychart account to access their medical information. You can view, print, or download information to your computer. Should you need to formally request records, you can do so directly through MyChart via the Document Center. Have a MyChart question? Please call (614) 355-0852.
By email:
Your completed Medical Record Request Form can be emailed to MedicalRecordRequests@NationwideChildrens.org.

In person (for urgent requests only):

Turnaround Time For Requests
All requests, whether delivered in-person, by mail, email, or via fax, could take up to 30 days to complete. Same day requests will be reviewed to determine if the request can be completed same day. Nationwide Children’s does charge a flat fee of $6.50 for each copy of the requested records, unless extraordinary circumstances apply. There is no fee associated with obtaining an immunization records or list of visit dates. Once your request is complete, we will contact you to collect payment, and the records will be released via the method requested on your Medical Record Request Form.
How to Request an Amendment to Your Medical Records
You have the right to request to amend health information we maintain about you or your child if you believe the health information is inaccurate or incomplete. Click here to complete a Medical Record Amendment Request form.
