The Partners for Kids Care Navigation Program improves the lives of children and their families by making health care easy to use. When a child is enrolled, a Care Navigation team member is assigned to work with schools, insurance companies, community groups and the health care team to help the child get what he or she needs.
Which patients best benefit from Care Navigation services?
- Children and families with complex medical problems or special health care needs.
- Families who have many people involved in their child’s health care including specialists, homecare nurses and schools.
- Families who frequently visit their primary care provider, emergency room, or hospital.
