The Intake Coordinator will ask you to explain the presenting problem and your concerns. The Intake Coordinator will gather pertinent patient and parent/guardian contact information. i.e.: Phone number, current address, client’s information as well as financial information.

Mandated reporters may call CCFA to schedule an appointment, but also are required to contact the appropriate Children Services agency. Learn more about mandated reporting and view a directory of agencies.
