To enter the Center for Healthy Weight and Nutrition Assessment Clinic, it is necessary for the patient to have a Body Mass Index of 95% or greater and be referred by a physician.
Physician offices may use our online service to Request an Appointment to have patients evaluated at the Center for Healthy Weight and Nutrition. In addition, please include growth charts with recent weights and heights, any labs, test results and letters.

Children under 6 will be scheduled to see a Recreational Therapist and Registered Dietitian.
Once a referral is received, the Center will call patients over 6 to schedule their first appointment called Assessment #1. After assessment #1, patients will schedule Assessment #2 with a Recreational Therapist and Registered Dietitian. Once Assessment #2 is complete patients will need to schedule Assessment #3 with our team Psychologist.
In scheduling appointments, the Center will attempt to contact you three times, if unsuccessful then a letter will be give to the referring physician stating the attempts.
Children and adolescents that do not meet the 95% BMI can be seen by Nationwide Children’s Clinical Nutrition Department.
Many insurance plans require authorization from the referring physician in order for the patient's care to be covered. Once all necessary referral and authorization information is received, the patient appointments will be scheduled.
