Rehabilitation Clinic

The Rehabilitation Clinic is designed to evaluate your child’s progress since discharge and can give you the opportunity to ask your specialists questions.

• Approximately eight to ten weeks after discharge from the Rehabilitation Unit, your child will be scheduled for a follow-up appointment in the Rehabilitation Clinic.
• Your child may be seen by an Occupational Therapist (OT), Physical Therapist (PT), Speech Therapist (SP), Therapeutic Recreation Specialist (TR), and or a Psychologist, in addition to the Rehab physician and nurse.
• The clinic appointment may last approximately two to three hours. Please eat lunch before coming to the clinic.
• Please bring any equipment you were given at discharge (i.e. hand/foot splint, wheelchair, walker, cane or crutches).

Insurance Coverage for the Clinic Visit

Prior authorization may be needed from your insurance company before coming to the clinic. Please check with your primary care physician or your insurance company in advance to see if you need a referral for the Rehabilitation Clinic (includes OT, PT, Speech, Therapeutic Recreation, Psychology and medical services).

The Rehabilitation Clinic is located on the first floor of Outpatient Care, to the left of the information desk next to the outpatient pharmacyc, in the clinic area labeled: Physical Medicine Clinic 1A. Use the 555 S. 18th Street entrance by the Visitor Parking Garage.

Clinic Referrals and Scheduling

The Rehabilitation Clinic accepts referrals from primary care physicians and pediatric specialists. To make a referral, there are two options:
