Interested candidates should visit the following website and click on “How To Apply” for a checklist of all items necessary to apply for both the Masters program at The Ohio State University, and for the residency at any or all of the five hospitals associated with this program, including Children’s Hospital:

When you get to the College of Pharmacy’s Application, they give you the option to check each hospital with which you want to share your information. Please be sure to check Children’s Hospital in addition to the other hospitals where you wish to pursue candidacy in order to apply to our program.
