Interested candidates should submit the following items to the Program Director no later than January 15:
- Letter of Intent – one page letter explaining why the candidate is interested in the position and what qualifications he/she has that are not fully captured within the remaining application packet
- Curriculum vitae
- College transcript(s)
- Three letters of recommendation
- Children’s Hospital Employment Application
An on-site interview is required. Application materials for the PGY1 Pharmacy Practice Residency (with emphasis in community care) should be mailed to:

Master of Science and PGY1/PGY2 Residency in Health-System Pharmacy Administration
Interested candidates should visit the following website and click on “How To Apply” for a checklist of all items necessary to apply for both the Masters program at The Ohio State University, and for the residency at any or all of the five hospitals associated with this program, including Children’s Hospital:

When you get to the College of Pharmacy’s Application, they give you the option to check each hospital with which you want to share your information. Please be sure to check Children’s Hospital in addition to the other hospitals where you wish to pursue candidacy in order to apply to our program.
An on-site interview is required. Admission to this program is contingent upon acceptance into the Masters Degree program.
