The Family Advisory Council at Nationwide Children's Hospital is proud to award an annual recognition to an employee, health care provider or department of Nationwide Children's who represents outstanding efforts to promote excellence in family-centered care. This annual award is presented at the Annual Employee Recognition Dinner. Deadline for nominations is December 6, 2013.

Please let us know why you feel this person or department best exemplifies the core concepts of Family Centered Care: Dignity and Respect, Information Sharing, Participation in Decision Making and Collaboration. Please list examples of how they have made a difference in your child's life and the life of others.
