Nationwide Children’s Hospital offers MyChart, a web-based and mobile resource for families, to access medical information for your child. This tool is available for both inpatients and outpatients and some of the features include:

If you are a patient and over 13 years old, you may request an activation code for yourself.
Parents can access the medical records of all family members seen at Nationwide Children's Hospital from one "proxy" account.
If you previously had access to your medical record through myChildren’s and your account has been deactivated, you will need to request a new activation code from your clinic and complete the signup process again.
