My Birthday Wish. Give the Gift of Hope.

Close your eyes!  Make a wish!  And give the gift of hope!  In lieu of receiving birthday gifts, register to participate in the “My Birthday Wish” program and ask friends and family to make a charitable donation in your honor to the patients and families at Nationwide Children’s Hospital.  We will get you started by providing an e-mail and set of insert cards announcing your participation and providing your friends and family instructions on how to give.  In appreciation, we will also provide you with a Birthday Kit which includes a set of thank you note cards and a special treat!

To register your birthday, complete the online form below or click here to print and mail the form.  Then, simply tell your family and friends to visit www.NationwideChildrens.org/BirthdayGifts to make a gift in honor of your special occasion.  And don’t forget to make your own personal donation to support children’s health in our community.
