Updated X-Ray Ordering Script Requirements

Starting in 2015, Nationwide Children’s Radiology Department has updated the x-ray ordering script requirements needed to complete an examination. These requirements must be met in full in order to efficiently complete the patient’s examination. The information that needs to be included by the ordering practitioner includes:

  • Patient name and date of birth
  • Exam requested
  • Reason for exam
  • Dated and signed by the ordering licensed practitioner

Any scripts that may be missing any of this information will require verbal authorization and a subsequent completed script faxed to the radiology department in which the patient is being seen before the x-ray(s) can be taken.
