Pharmacy and Therapeutics Update (May)

Ethanol lock therapy – revised guidelines for use were APPROVED and will be posted on the Pharmacy Department intranet site.

Pediarix (DTaP, IPV, Hepatitis B) vaccine was ADDED to the Formulary to provide consistency with the vaccines used by the Vaccines for Children program. Pentacel (DTaP, IPV, Hib) will no longer be on the Formulary.

The revised Surgical Prophylaxis: Dosages for Pre-Operative and Intra-Operative Antibiotics document was APPROVED and will be posted in the perioperative areas and on the Pharmacy Department intranet site.

Anthralin 1% topical cream was ADDED to the Formulary. The complete monograph will be posted on the Pharmacy Department intranet site.

Levonorgestrel IUD (Skyla) was ADDED to the Formulary. The complete monograph will be posted on the Pharmacy Department intranet site.

Valacyclovir was ADDED to the Formulary. The complete monograph will be posted on the Pharmacy Department intranet site.
