Pharmacy and Therapeutics Update (June)

The following actions were taken at the May 2 Pharmacy & Therapeutics Committee meeting. For more detailed information, contact Shelly Morvay at Shelly.Morvay@NationwideChildrens.org.

Automated dispensing cabinet override lists were APPROVED for J4NICU, C4NICU, Riverside NICU, H08A and the Emergency Department.

Medication stock lists were APPROVED for Primary Care Network, Child Advocacy Center for Family Safety and Healing, Interventional Radiology and Dublin Sports Medicine.

Guanfacine extended release will remain on the Formulary.

Daptomycin was ADDED to the Formulary. The complete monograph will be posted on the Pharmacy Department intranet site. Prescribing is restricted to Infectious Diseases Service or Infectious Diseases Consult.

The revised Medication Order Adjustments by Pharmacists policy was APPROVED. Pharmacists are now permitted to discontinue per pharmacist modification the oldest saline flush order when multiple identical saline flush orders are prescribed for an inpatient in Epic.

Drug cards were APPROVED for decitabine (non-formulary) and zanamivir (investigational). The drug cards will be posted in the online drug card database.
