MyChart Update: New Parent & Guardian Policy for Patients Age 13 and Older

Please review the new MyChart policy for parent/guardian’s access to MyChart for patients age 13 and older. It can be found on Anchor under Hospital Policies Section 075. Your patient’s parent/guardian may ask you about this policy. An SBAR was sent on 6/4/14 that highlighted parts of the policy:

  • Patients age 13 and over are able to request their own personal MyChart account.
  • Legal guardians of patients under age 13 may obtain proxy access to the patient’s MyChart account regardless of whether the patient is present at the time of the request.
  • Legal guardians of patients age 13 and over may only obtain proxy access to the patient’s MyChart account if the patient is present at the time of the request and the patient signs an acknowledgement form (Form OCC-965a), which acknowledges that the guardian may be able to determine from the information in MyChart that the patient received a service that the patient obtained without the consent of the guardian. If the patient refuses to sign the acknowledgement form, the guardian’s proxy access will not be activated.
  • If a guardian received proxy access prior to the patient turning 13 years old, at the patient’s first appointment after the patient’s 13th birthday, NCH registration staff will discuss MyChart access with the patient and request that the patient sign Form OCC-965a. If the adolescent refuses to sign the acknowledgement form, the guardian’s proxy access will be deactivated.
  • Patients age 18 and over may grant other adults proxy access to the patient’s MyChart account by completing Form OCC-965b.
