CareLink is a web-based tool that allows primary care providers (and associated staff) to view information in the Nationwide Children's Hospital Epic patient chart for mutual patients. CareLink is used extensively amongst primary care groups and has more than 6,000 log-ins per month.
CareLink users have always been able to set up alert notifications for certain patient activity. These notifications have included:
Now, two new notifications have been added to help primary care practices track patient activity:
ED/Urgent Care notifications — This is a list of patients who have been, or are currently, in the Nationwide Children’s ED or one of the Urgent Care sites. The list includes date/time of admission and date/time of discharge OR the patient’s current inpatient location (if that patient was admitted from one of these facilities). Providers and staff can then enter the charts of the patients on the list to view current status and pertinent notes.
Outpatient Notifications — When a patient visits a Nationwide Children’s specialist or clinic, and the charting is completed for that visit, a notification is sent to the primary care CareLink user. This allows for the primary care team to see when a patient has seen a specialist and when the charting is complete for that visit. Opening the patient chart takes the user to the note for that visit.
These notifications are in separate folders in every CareLink user’s “In Basket.” If a CareLink user wants to receive the notifications through an outside email, this can be set up too. These email notifications are sent out only once per day.
