Craniofacial and Special Care Orthodontic Fellowship Application Process

Application Qualifications

  • Successful completion of ADA-accredited Advanced Specialty Education Program in Orthodontics and Dentofacial Orthopedics by hire date.
  • Board-eligible by the American Board of Orthodontics.
  • Excellent oral and written communication skills for effective communication of information and positive interpersonal skills; ability to function in an interdisciplinary environment.
  • Completion of a research project or thesis preferred.

Application Information

Applications must be submitted no later than January 1, 2014. Once applications have been submitted and reviewed, candidate interviews will be scheduled.

Interested applicants should provide the following information by mail or email to the program coordinator:

  • Craniofacial and Special Care Orthodontic Fellowship Application
  • Cover Letter
  • Curriculum Vitae (include anticipated graduation date)
  • 3 letters of recommendation (one of which must be from your residency Program Director)
  • Transcript for dental training (scanned/unofficial copy is acceptable)
  • Current transcript for specialty training
