Biography

Dr. Gariepy is an attending pediatric gastroenterologist at Nationwide Children’s, a Principal Investigator in the Center for Cell and Developmental Biology in the Research Institute at Nationwide Children’s and an Associate Professor of Pediatrics at The Ohio State University College of Medicine. She has received funding from the National Institutes of Health for her research program focusing on the molecular signals controlling development of the nervous system in the intestines. She is the Gastroenterology representative to the Council of the Society for Pediatric Research and a member of the editorial board of the Journal of Pediatric Gastroenterology and Nutrition. She has published numerous research articles, presented her research at international meetings, and authored several book chapters. These include chapters in Sleisenger & Fortrans’ Gastrointestinal and Liver Disease; Molecular Basis of Inborn Errors of Development: The Molecular Basis of Clinical Disorders of Morphogenesis; Pediatric Gastrointestinal Disease: Pathophysiology, Diagnosis, and Management; and Physiology of the Gastrointestinal Tract. Dr. Gariepy is named among the “Best Doctors in America.” Her clinical interests include congenital abnormalities of the gastrointestinal tract and genetic disease.

  • The Gariepy lab studies the development of the enteric (intestinal) nervous system (ENS) using and animal model of the most common developmental abnormality of the ENS, Hirschsprung disease. Hirschsprung disease is the congenital absence of a portion of the ENS. It occurs in approximately 1 in 5000 births worldwide and leads to neonatal intestinal obstruction and death. Treatment consists of surgical removal of the affected intestine. Hirschsprung disease is the underlying diagnosis in 7% of small bowel transplants in the United States. Disabling symptoms caused by chronic alteration of ENS function occur in many disease. These include diabetic neuropathy, post-infections irritable bowel syndrome, schlerodema, disorders of the central nervous system (such as cerebral palsy, Parkinson’s disease and Alzheimers disease), amyloidosis, porphyria, Chagas disease, mitochondrial cytopathies, inflammatory bowel disease (Crohn’s disease and ulcerative colitis), congenital anomalies of the intestine, and necrotizing enterocolitis. Selective neurodegeneration in the ENS may also occur with aging and contribute to the increased frequency of constipation, dysphagia and gastroesophageal reflux seen in the elderly. Through improved understanding of the normal development and structure of the ENS and through the isolation and characterization of the stem cells of the ENS, we hope to contribute to improved diagnosis and treatment of these conditions. Mutation in many different genes has been linked to normal development of the ENS. The most common of these are in the tyrosine kinase receptor RET and the G-protein coupled receptor EDNRB. The ENS is derived from neural crest cells that invade the foregut and colonize the developing intestine in a cranial to caudal progression. Dr. Gariepy has shown that EDNRB expression on the neural crest cells is critical for normal colonization of the distal gut. She has also demonstrated that the EDNRB-deficient Hirschsprung rat harbors ENS stem cells in the proximal intestine. How the two receptors interact to support normal ENS development and how mutations in RET and EDNRB impair the ability of the ENS precursors remains unclear. We are interested in understanding the mechanisms controlling normal ENS development using a variety of molecular biologic, histologic, physiologic, and survival surgical techniques. We are examining the intracellular and extracellular events triggered by activation of RET and EDNRB on ENS stem cells which we can isolate prospectively from the rat intestine. We are also working to establish the therapeutic potential of ENS stem cells in disorders of the ENS through transplantation.
